چکیده

Transplantation of solid organs is the treatment of choice for most patients with end-stage organ diseases. In the absence of pharmacological immunosuppression, recognition of foreign (allogeneic) histocompatibility proteins expressedondonorcellsby therecipient’s immunesystem results in rejection of the transplanted tissue(s). One-year renal transplant survival is now routinely over 90% in most centres, largely the result of improvements in immunosuppressive drugs. In this article, we review commonly used immunosuppressive medications and discuss their pharmacological modes of action. Given that long-term graft outcomes remain poor despite improvements in early transplant survival, we discuss, in addition, novel experimental strategies for the induction of tolerance to transplanted tissues that have translational relevance to human organ recipients.

Immunosuppression with mycophenolate mofetil after penetrating keratoplasty: our clinical experience

Penetrating keratoplasty (PKP) has emerged as the most common form of solid tissue transplantation over the past 30 years1.This relative success of PKP is attributable to continued advances in surgical techniques, equipment, ocular pharmacology and immunology, corneal storage, and eye banking procedures. For uncomplicated first grafts performed in avascular «low-risk» beds with only local immun...

Immunological Tolerance: Therapeutic Induction

Immunological tolerance can be defined as an unresponsiveness of T and/or B cells to self antigens. Low responsiveness of T cells and/or B cells to self antigens could be termed partial tolerance. Protection from the development of autoimmune diseases and acceptance of transplanted organs or tissue are due to effective tolerance to self antigens and alloantigens, respectively. The breakdown of ...

Immunological tolerance-related genes in a spontaneous tolerant model of rat liver transplantation explored by suppression subtractive hybridization.

Natural immunological tolerance can be induced in certain types of allogeneic liver transplantation in rats. To screen for genes associated with the induction of tolerance, suppression subtractive hybridization was performed in the rat liver transplantation model between a DA donor and PVG recipient combination where spontaneous immunological tolerance is known to occur without any immunosuppre...

Aspects of Corneal Transplant Immunology

249 Eduard Zirm performed the first successful allogeneic corneal transplant in 1905,[1] making this corneal graft a pioneering event in early organ transplantation.[2] Despite this long history, our knowledge of corneal immune function and graft rejection pathophysiology and its prevention are lagging in comparison to solid organ transplantation. This is, to some extent, due to a misunderstand...

Chronic renal transplant loss

Ever since the inception of kidney transplant programs the short-term patient and graft survival results have improved progressively, principally because of better management of acute rejection episodes and decreased mortality from infectious complications. However, the rate of annual graft attrition after the first post-transplant year has virtually remained unchanged [1, 2]. About half to thr...
